Dawson Street Veterinary Clinic offers our patient forms online so you can complete them in the convenience of your own home or office.

COVID-19 Screening Form

If you are feeling ill, have been out of the country, or may have been possibly exposed to someone with COVID-19 in the past 14 days, please AVOID entering the clinic as we would like to protect all of our clients and employees.
If you have an appointment, and are not feeling well, please call us to CANCEL & RESCHEDULE your appointment to when you are cleared from quarantine as directed by BC health authorities.
There is also NO CANCELLATION fee for cancelling your appointment, we would appreciate it you could please call us to give us as much notice as possible to allow us to offer your appointment to another patient.
This allows us to record pet(s) and owner information into our management software so that we can effectively and efficiently provide the service you deserve!

Boarding Consent Form
This form is required for each boarding patient. It provides us with information of duration of stay, time of arrival/pick up, emergency contact, vaccine status requirements, and items (food, medication, toys, bedding, leashes, etc.) brought with the pet.
An up-to-date vaccination record is required. Please contact your veterinarian for the certificate. If your pet is due for any vaccines, they can be done here at Dawson Street Veterinary Clinic during their stay.
